Why we exist

The system made you wait. We decided you shouldn't have to.

For too long, getting help with your weight and your health meant weeks of waiting for an appointment, fighting insurance for approvals, and prices that changed every time your dose did. People who were ready to start were told to wait. Many simply gave up.

We started CURYVO to remove every one of those walls. We brought together licensed U.S. providers, trusted U.S. pharmacies and a care team that actually answers, then wrapped it in an experience that takes minutes. No waiting rooms. No insurance hoops. No surprise bills.

What hasn't changed is the medicine. Every treatment is decided by a real, licensed provider who reviews your health history and only prescribes if it's right for you. Fast, yes. But never careless.

CURYVO is a U.S. telehealth platform for doctor-prescribed weight loss and longevity care. We connect you with independent licensed U.S. providers and licensed partner pharmacies, and ship your medication straight to your door. CURYVO INC. is a Delaware corporation.

It depends on your state and treatment. Your provider may review your assessment online or meet you by video. Messaging with your provider and care team is always included.

Most assessments are reviewed within 2–3 hours. Sometimes it can take longer, but as a rule a review is completed within 24 hours. Approved orders ship the next business day.

GLP-1 (semaglutide) and Tirzepatide for weight loss, as injections or as needle-free Weight Loss Melts, plus Sermorelin and NAD+ for longevity. Your provider recommends the treatment, format and dose that fit you.

Melts are small orally disintegrating tablets that dissolve right on your tongue. No needles and no water needed. They are available as GLP-1 Melts (semaglutide) and Tirzepatide Melts (GLP-1 + GIP).

Both are prescribed and monitored by the same licensed providers. Melts are needle-free and easy to take on the go; injections are taken on the schedule your provider prescribes. Your provider helps you choose based on your goals, history and comfort level.

GLP-1 (semaglutide) acts on one hormone pathway that helps regulate appetite and blood sugar. Tirzepatide is dual-action and activates two pathways, GLP-1 and GIP. Your provider decides which one is appropriate for you.

Yes, if your provider agrees it is appropriate. Just message your care team and your provider will review the change with you.

The most common side effects of GLP-1 medications are gastrointestinal, such as nausea, constipation or diarrhea, and they often ease as your body adjusts. Providers typically start with a low dose and increase it gradually. If symptoms are severe or persistent, contact your care team right away.

Terms of Use

Last updated: October 29, 2026CURYVO INC.Delaware, USA

These Terms of Use ("Terms") are a binding agreement between you and CURYVO INC., a Delaware corporation doing business as CURYVO ("CURYVO," "we," "us," or "our"). They govern your access to and use of curyvo.com, our patient portal, communications and related services (together, the "Services").

Please read Section 18 carefully. It contains a binding arbitration agreement and a class action waiver that affect how disputes are resolved.

01Acceptance of these Terms

By accessing the Services, creating an account, completing an online assessment or placing an order, you confirm that you have read, understood and agree to these Terms, our Privacy Policy, our Telehealth Informed Consent, our Refund & Cancellation Policy and our Compounded Medication Disclosure, each incorporated by reference. If you do not agree, do not use the Services.

02Who we are and what we do

CURYVO is an online healthcare platform that connects eligible U.S. patients with independent licensed medical providers and partner pharmacies. CURYVO INC. is not a medical provider or a pharmacy and does not practice medicine or pharmacy.

Medical services offered through CURYVO are provided by independent, state-licensed physicians, physician assistants, nurse practitioners and other healthcare professionals (the "Providers"), who may practice through independently owned professional entities. Medications are dispensed by independently licensed U.S. pharmacies (the "Partner Pharmacies").

  • Providers exercise their own independent clinical judgment. CURYVO does not control, direct or interfere with the professional judgment of any Provider or pharmacist.
  • Using the Services creates a provider–patient relationship only between you and the Provider who treats you. No provider–patient relationship is formed with CURYVO INC.
  • Completing an assessment does not guarantee that you will receive a prescription. A Provider will only prescribe a treatment if they determine it is medically appropriate for you.

03Eligibility

  • You must be at least 18 years old and able to form a binding contract.
  • You must be physically located in a U.S. state where the Services are available at the time of your consultation and at the time your medication is delivered.
  • You must provide complete, accurate and current information about your identity and health. Providing false or incomplete information may put your health at risk and is a breach of these Terms.
  • We or a Provider may ask you to verify your identity, upload a photo, complete a video visit or provide lab results before treatment can be prescribed.

04Not for emergencies

The Services are not for medical emergencies. If you think you may be having a medical emergency, call 911 or go to the nearest emergency room immediately. If you are having thoughts of harming yourself, call or text 988 (Suicide & Crisis Lifeline).

The Services do not replace your relationship with a primary care physician. We encourage you to keep your primary care provider informed about any treatment you receive.

05Your account

  • You are responsible for maintaining the confidentiality of your login credentials and for all activity under your account.
  • Your account is personal to you. You may not create an account for, or order medication on behalf of, another person.
  • Notify us immediately at care@curyvo.com if you suspect unauthorized access to your account.

06Assessments, visits and prescriptions

Your care begins with an online medical assessment that typically takes about three minutes. A Provider reviews your answers and may contact you by secure message, phone or video with follow-up questions. Review times vary by state, Provider availability and the completeness of your information; many assessments are reviewed within hours, but timing is not guaranteed.

  • If a Provider determines that a treatment is appropriate, they may issue a prescription to a Partner Pharmacy.
  • Where permitted by law, you may ask your Provider to send your prescription to a pharmacy of your choice. Pricing on CURYVO applies only to medication dispensed by Partner Pharmacies.
  • You agree to follow your Provider's instructions, read all medication information, report side effects promptly and respond to required check-ins. Your Provider may pause, adjust or discontinue treatment at any time based on their clinical judgment.
  • Providers do not prescribe controlled substances through the Services.

07Medications and compounding

Some treatments available through the Services are compounded medications prepared by state-licensed pharmacies for an individual patient based on a valid prescription. Compounded medications are not approved by the U.S. Food and Drug Administration (FDA). The FDA does not review compounded drugs for safety, effectiveness or quality. Compounded medications are not generic versions of, and are not the same as, FDA-approved brand-name drugs. See our Compounded Medication Disclosure.

Medication availability may change at any time because of supply, regulatory or legal developments. If a medication becomes unavailable, we will notify you and your Provider will discuss your options.

08Pricing, payment and recurring billing

  • All-inclusive pricing. Unless clearly stated otherwise at checkout, your program price includes your Provider evaluation, the medication dispensed by a Partner Pharmacy, standard shipping and ongoing messaging with your care team.
  • Dose-based pricing. Prices may vary by medication, strength and dose. Prices shown as "starting at" reflect the lowest available dose. The price of your plan is shown before you pay, and if a dose change prescribed by your Provider changes your price, we will tell you before you are charged.
  • No insurance required. The Services are self-pay. We do not bill Medicare, Medicaid or private insurance, and you agree not to submit claims for the Services to any government healthcare program.
  • Recurring billing. If you choose a subscription or multi-month plan, you authorize us and our payment processors to charge your payment method automatically at the interval shown at checkout until you cancel. We will disclose the price, billing interval and how to cancel before you purchase.
  • Buy now, pay later. If you pay with Klarna, Affirm or another third-party financing provider, that provider's terms govern your financing, and the provider (not CURYVO) is responsible for the credit decision. Payment options through Affirm are subject to an eligibility check, may not be available in all states, and are provided by Affirm's lending partners (see affirm.com/lenders). 0% APR installment availability depends on eligibility and the plan you select.
  • Price changes. We may change prices for future orders. We will notify you before any price change applies to an active subscription, and you may cancel before the new price takes effect.
  • Applicable taxes, if any, are shown at checkout.

09Cancellation and refunds

You can cancel at any time before your next order is processed. Refunds are governed by our Refund & Cancellation Policy. Because of federal and state pharmacy laws, dispensed medications cannot be returned.

10Shipping and delivery

  • We ship only to addresses within the United States, in states where the Services are available.
  • Approved orders typically ship the next business day after your Provider's approval and the Partner Pharmacy's processing. Delivery times depend on the carrier and are estimates, not guarantees.
  • Some medications require temperature control. Please refrigerate them as directed on delivery and contact us right away if a package arrives damaged, warm or incorrect.
  • You are responsible for providing an accurate shipping address. Title and risk of loss pass to you on delivery to the carrier, except where applicable law provides otherwise.

11Electronic communications and text messages

You consent to receive communications from us, Providers and Partner Pharmacies electronically, including by email, in-app messages and text message, and agree that these communications satisfy any legal requirement that they be in writing.

SMS program. By providing your mobile number and opting in, you agree to receive recurring automated text messages related to your account, care, orders and shipping, and, if you separately opt in, marketing messages. Consent to marketing messages is not a condition of purchase. Message frequency varies. Message and data rates may apply. Reply STOP to opt out at any time and HELP for help, or contact care@curyvo.com. Carriers are not liable for delayed or undelivered messages.

12Acceptable use

You agree not to:

  • provide false information, impersonate anyone or use another person's identity or payment method;
  • resell, share, give away or transfer any medication you receive;
  • copy, scrape, reverse engineer, interfere with or disrupt the Services or their security features;
  • use the Services for any unlawful purpose or in violation of these Terms;
  • harass, threaten or abuse Providers, pharmacists or CURYVO staff.

We may suspend or terminate access for violations.

13Intellectual property

The Services and all content, design, text, graphics, logos, video and software on them are owned by CURYVO INC. or its licensors and are protected by U.S. and international intellectual property laws. CURYVO and the CURYVO logo are trademarks of CURYVO INC.. We grant you a limited, revocable, non-exclusive, non-transferable license to use the Services for your personal, non-commercial use in accordance with these Terms.

If you send us feedback or suggestions, we may use them without obligation to you.

14Third-party services

The Services may integrate with or link to third-party services (for example payment processors, financing providers, shipping carriers, video platforms and lab partners). We are not responsible for third-party services, and your use of them is subject to their own terms and privacy policies.

15Disclaimers

Information on the Services is provided for general informational purposes and is not medical advice. Individual results vary, and no specific outcome is promised or guaranteed.

TO THE FULLEST EXTENT PERMITTED BY LAW, THE SERVICES ARE PROVIDED "AS IS" AND "AS AVAILABLE," WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, TITLE AND NON-INFRINGEMENT. CURYVO DOES NOT WARRANT THAT THE SERVICES WILL BE UNINTERRUPTED, ERROR-FREE OR SECURE. CURYVO IS NOT RESPONSIBLE FOR THE CLINICAL DECISIONS OF PROVIDERS OR THE DISPENSING DECISIONS OF PHARMACISTS.

16Limitation of liability

TO THE FULLEST EXTENT PERMITTED BY LAW, CURYVO AND ITS OFFICERS, DIRECTORS, EMPLOYEES AND AGENTS WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, EXEMPLARY OR PUNITIVE DAMAGES, OR FOR ANY LOSS OF PROFITS, DATA OR GOODWILL, ARISING OUT OF OR RELATED TO THE SERVICES. OUR TOTAL LIABILITY FOR ANY CLAIM ARISING OUT OF OR RELATING TO THE SERVICES WILL NOT EXCEED THE GREATER OF (A) THE AMOUNTS YOU PAID TO CURYVO IN THE TWELVE (12) MONTHS BEFORE THE EVENT GIVING RISE TO THE CLAIM OR (B) ONE HUNDRED U.S. DOLLARS (US $100).

Some states do not allow the exclusion or limitation of certain damages, so some of these limitations may not apply to you. Nothing in these Terms limits liability that cannot be limited under applicable law.

17Indemnification

You agree to indemnify and hold harmless CURYVO and its officers, directors, employees and agents from any claims, losses, liabilities and expenses (including reasonable attorneys' fees) arising out of your breach of these Terms, your misuse of the Services or your violation of any law or the rights of a third party.

18Dispute resolution, arbitration and class action waiver

Informal resolution first. Before starting any formal proceeding, you agree to contact us at care@curyvo.com with a written description of your claim, and we will try in good faith to resolve it within 60 days.

Binding arbitration. Except for claims that qualify for small claims court and claims for injunctive relief relating to intellectual property, any dispute arising out of or relating to these Terms or the Services will be resolved by binding individual arbitration administered by the American Arbitration Association (AAA) under its Consumer Arbitration Rules. The Federal Arbitration Act governs this provision. Arbitration may take place remotely or in the county where you live. Fees will be allocated as provided in the AAA Consumer Rules.

Class action waiver. You and CURYVO agree that each may bring claims against the other only in an individual capacity, and not as a plaintiff or class member in any purported class, collective or representative proceeding. The arbitrator may not consolidate claims of more than one person.

Your right to opt out. You may opt out of this arbitration agreement by emailing care@curyvo.com within 30 days after you first accept these Terms, with your name, address and a clear statement that you opt out of arbitration.

This Section does not apply to claims of medical malpractice against a Provider, which are governed by applicable state law.

19Governing law and venue

These Terms are governed by the laws of the State of Delaware and applicable U.S. federal law, without regard to conflict-of-law rules. Subject to Section 18, any action not subject to arbitration will be brought exclusively in the state or federal courts located in the State of Delaware, and you consent to their jurisdiction.

20Changes to these Terms

We may update these Terms from time to time. If we make material changes, we will notify you by email or through the Services before they take effect. Your continued use of the Services after the effective date means you accept the updated Terms.

21Suspension and termination

You may close your account at any time by contacting care@curyvo.com. We may suspend or terminate your access if you violate these Terms, if required by law, or if continuing would create risk to you, Providers or others. Sections that by their nature should survive termination will survive, including payment obligations, disclaimers, limitations of liability, indemnification and dispute resolution. Your medical records will be retained as required by law.

22General

  • These Terms and the policies referenced in them are the entire agreement between you and CURYVO about the Services.
  • If any provision is found unenforceable, the remaining provisions remain in effect.
  • Our failure to enforce a provision is not a waiver of it.
  • You may not assign these Terms without our consent. We may assign them in connection with a merger, acquisition or sale of assets.
  • We are not liable for delays or failures caused by events beyond our reasonable control.

HIPAA Notice of Privacy Practices

Last updated: October 29, 2026CURYVO INC.Delaware, USA

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

This Notice is provided on behalf of the independent licensed Providers who deliver care through the CURYVO platform (each a "Practice") and applies to the protected health information ("PHI") they create or receive about you. CURYVO INC. supports the Practices as a HIPAA business associate and is bound by the same protections when it handles your PHI. Partner Pharmacies that dispense your medication are separate covered entities and provide their own notices.

01Your rights

When it comes to your health information, you have the right to:

  • Get a copy of your medical record. Ask to see or get an electronic or paper copy of your record. We will usually provide it within 30 days and may charge a reasonable, cost-based fee.
  • Ask us to correct your record. If you think information is incorrect or incomplete, you can ask us to amend it. We may say no, but will tell you why in writing within 60 days.
  • Request confidential communications. Ask us to contact you in a specific way (for example, a different email or phone number). We will say yes to all reasonable requests.
  • Ask us to limit what we use or share. We are not required to agree, except that if you pay for a service out of pocket in full, you can ask us not to share that information with your health insurer for payment or operations.
  • Get a list of those with whom we have shared information for up to six years before your request, other than for treatment, payment and health care operations and certain other disclosures.
  • Get a copy of this Notice at any time, including a paper copy on request.
  • Choose someone to act for you, such as a legal guardian or someone with medical power of attorney.
  • File a complaint if you believe your rights have been violated (see below).

02Your choices

  • You can tell us whether to share information with family, close friends or others involved in your care.
  • We will never sell your information or use it for marketing without your written permission.
  • If you are not able to tell us your preference, for example if you are unconscious, we may share your information if we believe it is in your best interest, or when needed to lessen a serious and imminent threat to health or safety.

03How we use and share your information

We typically use or share your health information in the following ways:

  • Treatment: to evaluate you, prescribe medication and coordinate your care with other professionals, including Partner Pharmacies.
  • Payment: to bill and get paid for services, including through payment processors.
  • Health care operations: to run our practice, improve care quality, train staff and contact you when necessary.

We are also allowed or required to share your information in other ways, usually for purposes that contribute to the public good, when we meet conditions in the law:

  • helping with public health and safety issues, such as reporting adverse reactions to medications or preventing disease;
  • doing research under strict privacy safeguards;
  • complying with the law, including with the Department of Health and Human Services;
  • responding to organ and tissue donation requests and working with a medical examiner or funeral director;
  • addressing workers' compensation, law enforcement and other government requests, including health oversight and special government functions;
  • responding to lawsuits and legal actions.

Most uses and disclosures of psychotherapy notes, uses for marketing and the sale of PHI require your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already relied on it.

04Reproductive health care information

We will not use or disclose PHI to investigate or impose liability on any person for seeking, obtaining, providing or facilitating lawful reproductive health care, and may require a signed attestation before disclosing PHI potentially related to reproductive health care for health oversight, judicial, law enforcement or coroner purposes, as required by federal law.

05Our responsibilities

  • We are required by law to maintain the privacy and security of your PHI.
  • We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.
  • We must follow the duties and privacy practices described in this Notice and give you a copy of it.
  • We will not use or share your information other than as described here unless you tell us we can in writing.

06Changes to this Notice

We can change the terms of this Notice, and the changes will apply to all information we have about you. The new Notice will be available on our website and on request.

07Complaints

You can file a complaint with us by contacting our Privacy Officer at care@curyvo.com or (740) 562-8088, or by mail at the address below. Telehealth Informed Consent

Last updated: October 29, 2026CURYVO INC.Delaware, USA

Telehealth is the delivery of healthcare services using electronic communications, such as secure messaging, online questionnaires, photos, phone and video, when you and your provider are not in the same physical location. Before you receive care through CURYVO, please read this consent carefully. By completing an assessment, you confirm that you understand and agree to it.

01How telehealth works with CURYVO

  • You complete an online medical assessment and may upload photos, lab results or identification.
  • An independent licensed Provider, licensed in the state where you are located, reviews your information. Care may be asynchronous (the Provider reviews your information and responds later) or synchronous (live phone or video), depending on your state's requirements and the Provider's judgment.
  • If a treatment is appropriate, the Provider may send a prescription to a Partner Pharmacy, which ships your medication to you.
  • You can message your care team with questions throughout your treatment.

02Expected benefits

  • Convenient access to licensed Providers without travelling to an office.
  • Faster evaluation and follow-up compared with many in-person settings.
  • Ongoing support and easier communication with your care team.

03Possible risks and limitations

  • Information you provide may not be sufficient for the Provider to make an appropriate decision, and the Provider cannot perform a physical examination.
  • Delays in evaluation or treatment may occur because of technical problems or incomplete information.
  • In rare cases, security protocols could fail and cause a breach of the privacy of personal health information.
  • A lack of access to complete medical records may result in adverse drug interactions, allergic reactions or other judgment errors.
  • Your Provider may determine that telehealth is not appropriate for you and refer you to in-person care.

04Your responsibilities

  • Provide complete, honest and accurate information about your health, medical history, medications and allergies, and update it if anything changes.
  • Read all information provided with your medication and follow your Provider's instructions.
  • Report side effects or changes in your health promptly. If you experience a medical emergency, call 911.
  • Keep your primary care provider informed of the care you receive through CURYVO, and maintain regular in-person care as recommended.
  • Be physically located in a state where the Services are available when you receive care.

05Your rights

  • You may decline or stop telehealth services at any time without affecting your right to future care or treatment.
  • You have the right to request in-person care instead; CURYVO does not provide in-person care and can help direct you to local resources.
  • Your health information is protected under federal and state law, including HIPAA. See our HIPAA Notice.
  • You may request a copy of your medical records.

06Financial terms

Telehealth services through CURYVO are self-pay and are not billed to insurance. Pricing is described at checkout and in our Terms of Use.

Refund & Cancellation Policy

Last updated: October 29, 2026CURYVO INC.Delaware, USA

We want pricing and refunds to be as clear as our care. Here is exactly how billing, cancellations and refunds work at CURYVO.

01How billing works

  • Your price is all-inclusive: Provider evaluation, medication from a licensed U.S. Partner Pharmacy, standard shipping and ongoing care team support. There are no hidden fees.
  • Your payment is authorized when you check out. If you are on a recurring plan, you will be billed automatically at the interval shown at checkout until you cancel.
  • Prices may vary by medication and dose. If a dose change prescribed by your Provider changes your price, we will tell you before your next charge, and you can cancel at any time.

02If you are not approved

If a Provider determines that treatment is not appropriate for you, you will receive a full refund of any amount charged for that order.

03Cancelling your plan

  • You can cancel at any time, without a cancellation fee, from your account or by contacting us at care@curyvo.com or (740) 562-8088.
  • To avoid being charged for your next order, cancel before your next billing date shown in your account.
  • Once a prescription has been sent to the Partner Pharmacy for processing, that order cannot be cancelled.
  • Cancelling stops future orders. You keep access to your care team for questions about medication you have already received.

04Refunds after approval

  • Because of federal and state pharmacy regulations, medications that have been dispensed or shipped cannot be returned or resold, and orders that have entered pharmacy processing are not refundable.
  • If you are charged for an order after you cancelled before your billing date, we will refund that charge in full.
  • If you were charged in error, contact us and we will correct it promptly.

05Damaged, incorrect or missing orders

  • If your package arrives damaged, outside the required temperature range, incorrect or does not arrive, contact us within 7 days of the expected delivery date.
  • Please do not use medication that appears damaged or was not kept at the required temperature. We will arrange a replacement or a refund at no cost to you after reviewing the issue with the Partner Pharmacy and carrier.

06Refund timing and methods

  • Approved refunds are issued to your original payment method, usually within 5 to 10 business days, depending on your bank.
  • If you paid with Klarna, Affirm or another financing provider, the refund is processed through that provider and applied according to its terms.

Compounded Medication Disclosure

Last updated: October 29, 2026CURYVO INC.Delaware, USA

Some treatments prescribed through CURYVO are compounded medications. We believe you deserve to understand exactly what that means before you start.

01What compounding is

Compounding is the process by which a licensed pharmacist combines, mixes or alters ingredients to create a medication tailored to the needs of an individual patient, based on a valid prescription from a licensed provider. Compounded medications prescribed through CURYVO are prepared by state-licensed pharmacies in the United States that operate under federal and state pharmacy laws, including Section 503A of the Federal Food, Drug, and Cosmetic Act where applicable.

02FDA status

Compounded medications are not approved by the FDA. The FDA does not review compounded drugs for safety, effectiveness or quality before they are dispensed.

  • Compounded medications are not generic versions of, and are not the same as, FDA-approved brand-name medications. They may differ in formulation, dosage form and inactive ingredients.
  • Compounded GLP-1 and GLP-1/GIP medications are not the same as FDA-approved brand-name products containing related active ingredients.
  • CURYVO does not claim that any compounded medication is equivalent to, or has the same safety or effectiveness as, any FDA-approved drug.

03When a compounded medication may be prescribed

A Provider will prescribe a compounded medication only if they determine, in their independent clinical judgment, that it is medically appropriate for you based on your individual health information, for example because of a specific dosage, formulation or other clinical need. Your Provider will discuss your options with you, including FDA-approved alternatives where available.

04Safety information

  • Read all information provided with your medication and follow your Provider's instructions for dosing, storage and use.
  • Like all medications, compounded medications can cause side effects. Contact your care team promptly if you have questions or experience side effects. In an emergency, call 911.
  • You can also report side effects to the FDA's MedWatch program at 1-800-FDA-1088 or fda.gov/medwatch.

05Availability

The availability of compounded medications may change because of FDA actions, drug shortage determinations, state laws or supply conditions. If a medication you receive becomes unavailable, we will let you know and your Provider will discuss your options with you.

06Results

Individual results vary. Treatment works best alongside nutrition, physical activity and other lifestyle changes discussed with your Provider. No specific outcome is guaranteed.
